Abstract

ABSTRACT

Total protein measurement is a useful test in a variety of disorders. A decrease in total protein concentration can be detected in impaired protein synthesis in the liver, protein loss due to impaired renal function, intestinal malabsorption, or nutritional deficiencies. Elevated protein levels occur in chronic inflammatory disorders, liver cirrhosis, and dehydration. Total protein examination can use venous blood made from plasma or serum. Making plasma from venous blood will have an osmotic effect due to the addition of anticoagulants which cause water to leave the cells and enter the plasma, thus thinning the plasma and reducing its concentration. The choice of specimen type during sample preparation can influence the results of examining total protein levels. In the photometric test according to the Biuret method, protein forms a purple-blue complex with copper ions in alkaline solutions. Color absorbance is directly proportional to concentration. Research has been carried out on the comparison of total protein examination results in serum samples, EDTA plasma samples and citrate plasma samples. The data obtained was an average total protein yield in serum of 7.30%, in EDTA plasma samples of 7.90%, and in citrate plasma of 6.76%. The data was processed statistically. The results obtained were processed using statistics in the form of normality tests, homogeneity tests, Kruskal Walis post hoc test, Mann-Whitney test and followed with the t-paired test. It was found that the total protein examination results in each data group were significantly different. This shows that specimen selection influences the examination results. It is recommended that for total protein examination, the sample chosen is fresh serum.

Penurunan konsentrasi protein total dapat dideteksi pada gangguan sintesis protein di hati, kehilangan protein akibat gangguan fungsi ginjal, malabsorpsi usus, atau defisiensi nutrisi. Peningkatan kadar protein terjadi pada gangguan peradangan kronis, sirosis hati, dan dehidrasi. Pemeriksaan total protein dapat menggunakan darah vena yang dibuat plasma atau serum. Pembuatan plasma dari darah vena akan memberikan efek osmotik karena penambahan antikoagulan yang menyebabkan air meninggalkan sel dan memasuki plasma, sehingga menipiskan plasma dan menurunkan konsentrasi. Pemilihan jenis spesimen pada tahan persiapan sampel dapat mempengaruhi hasi dari pemeriksaaan kadar total protein, Pada uji fotometrik menurut metode biuret protein membentuk kompleks warna biru ungu dengan ion tembaga dalam larutan basa. Absorbansi warna berbanding lurus dengan konsentrasi. Telah dilakukan penelitian tentang perbandingan hasil pemeriksaan protein total pada sampel serum, sampel plasma EDTA, dan sampel plasma sitrat. Data yang didapat adalah rata-rata hasil protein total pada serum sebesar 7,30%, pada sampel plasma EDTA sebesar 7,90%, dan pada plasma sitrat sebesar 6,76%. Data tersebut diolah secara statistika Hasil yang didapat diolah dengan menggunakan statistika berupa uji normalitas, uji homogenitas, uji post hoc Kruskal Walis, uji Mann-Whitney dan dilanjutkan dengan uji t-paired test. Didapatkan hasil pemeriksaan protein total pada tiap kelompok data berbeda secara signifikan. Hal ini menunjukkan bahwa pemilihan specimen memengaruhi hasil pemeriksan. Disarankan untuk pemeriksaan protein total, sampel yang dipilih adalah serum segar. Kata kunci: kadar protein total, serum, plasma EDTA, plasma sitrat, metoda Biuret
